How To Medically Treat Nail Fungus - Comments Off

Warning: This article is for information only and can not be used to supplement your doctor’s advice. You should contact your doctor for nail fungus treatment.

Treatment Of Toe Nail Fungus

Medical Care:

Several years ago the medical management of OM was limited to topical therapy and 2 unreliable systemic drugs: griseofulvin and ketoconazole. Topical therapy is beneficial only for mild cases involving the very distal nail plate. The use of griseofulvin and ketoconazole is plagued by high relapse rates (70-85%), prolonged treatment regimens (10-18 mo for toenails), constant laboratory monitoring, and numerous adverse effects. The introduction of newer oral agents has revolutionized the medical treatment of OM and reduced potential adverse effects and drug interactions. As the rate of recurrence remains high, even with newer agents, the decision to treat should be made with a clear understanding of the cost and risks involved as well as the risk of recurrence..

Topical antifungals

The use of topical agents should be limited to cases involving less than half of the distal nail plate or for patients unable to tolerate systemic treatment. Agents include amorolfine (approved in other countries), ciclopirox olamine 8% nail lacquer solution, sodium pyrithione, bifonazole/urea (available outside the United States), propylene glycol-urea-lactic acid, the imidazoles, and the allylamines.

Topical treatments alone are generally unable to cure OM because of insufficient nail plate penetration. Ciclopirox solution has been reported to penetrate through all nail layers but has low efficacy when used as monotherapy. It may be useful as adjunctive therapy in combination with oral therapy or as prophylaxis to prevent recurrence in patients cured with systemic agents.

Oral therapy

The newer generation of oral antifungal agents (itraconazole and terbinafine) has replaced older therapies in the treatment of nail fungus. They offer shorter treatment regimens, higher cure rates, and fewer adverse effects. Fluconazole (not approved by the Food and Drug Administration [FDA] for treatment of nail fungus) offers an alternative to itraconazole and terbinafine. Derivatives of fluconazole may also be available soon. The efficacy of the newer antifungal agents lies in their ability to penetrate the nail plate within days of starting therapy. Recent evidence shows better efficacy with terbinafine than other oral agents.

To decrease the adverse effects and duration of oral therapy, topical and surgical treatments may be combined with oral antifungal management.

Surgical Care:

Surgical approaches to nail fungus treatment include surgical nail avulsion and matrixectomy by chemical or mechanical means.
Chemical removal by using a 40-50% urea compound should be reserved for patients with very thick nails or for those who may not tolerate mechanical avulsion.

Removal of the nail plate should be considered an adjunctive treatment in patients undergoing oral therapy.

A combination of oral, topical, and surgical therapy can increase efficacy and reduce cost.

Activity: Activity does not need to be limited during treatment, but patients should be educated about avoiding direct contact with high-risk areas in public places.

Get Ahead When You Build Your Own Computer - Comments Off

If you’ve been kicking around the idea of building your own computer, it actually isn’t a bad idea. It’s easier than you might think, and you can probably come out with a system that gives you more kick for your money, than you’d see in retail, or those made-to-order places. There are mainly two ways you’re going to see the benefits here:

1. You can see the money you’re going to save on this right up front. Just do a search on Cnet.com under “Compare Prices” for any random PC component you see in the stores, and you’ll know what I’m talking about. In most cases, you’ll cut out the full-retail markup, and then some. You’ll find brand new, sealed, in-box items lower online than you’ll ever find in any store.

2. The quality of the parts you’re getting is much better than what they put in pre-assembled, store-bought computers. You’ll know which brands mean quality. You could have a Creative sound card and a video card made by ATI or nVidia; the price difference is negligible, and we’re dealing with quality and performance standards that dwarf any no-name components they put in store bought systems, so they can keep their costs down. Many times, computer manufacturers like just use a cheap motherboard that has everything, your sound, video, etc., integrated into it; you get a third-rate version of everything.

Here are the main parts, or the basic building blocks for what you’ll need when you build your own computer.

1. A Motherboard. I talked about this a little earlier. All computers have one, but a good motherboard is just a fast connector that’ll hold lots of memory and extra slots for expansion components. Steer clear of motherboards that have “built-in” this or “integrated” that, because they’re almost always of lousy quality. It does help to have all your USB ports, keyboard plugs etc., on your motherboard, however. I have an onboard LAN that works pretty well, since a LAN is a LAN, but there were some driver issues with it when I put it together.

2. The Processor, or CPU. This is the “brain” of your computer. The kind of CPU that you can get depends on the kind of board that you picked out. The market is pretty much Intel Pentium 4 or AMD. Most people who are interested in higher power and long-term use tend to opt for the Intel.

3. Video Card. This is easy. Your motherboard either has a PCI Express video slot (newer and faster, but no real benefit yet) or an AGP 8X slot. Are you a hardcore gamer or someone who just uses a PC for a little email and word processing every now and then? This is the major question that’ll tell you the video card you need. Obviously the more intense and rigorous your video and graphics demands, the more video memory your card will need.

4. Memory. Memory or RAM (Random Access Memory) is easy too. The main size ram you’ll find that goes with what’s still the most common new computer motherboard size (the Pentium compatible, socket 478) is the standard DDR 184 pin memory stick. Whether you need DDR 333, DDR 400, or DDR 533 (mhz, frequency and speed really) depends on the board you chose. Some handle all of them, some handle just a few, so be sure to read that part of the box, (or online description).

5. Sound Card. This is probably one of the easiest parts to pick out and add. Most standalone sound cards you’ll see out there (the best way to go) are pretty good in the digital sound that they’ll give you. I would go with Creative Labs, who I mentioned earlier, because they’ve been around since the mid 80’s; they’ve got this right. Almost every one sold and made today is the PCI slot, the most common motherboard slot, so it’s pretty hard to pick a sound card that won’t work.

6. Hard Drive. This is your main storage on your PC. It holds all of your files, games, pictures, operating system, everything. I would make sure I had at least 160 gigs of space, even if I weren’t a power user. This sounds like a lot but you’ll find that just in everyday PC use, that space gets eaten up fast. There are a few quality names out there that most people know, like Western Digital and Seagate.

7. Chassis / Computer Case. It’s good idea to get a case that comes with the power supply already in it, so you won’t have to deal with the annoyance of buying one separately and fitting it in. Plus, the power supply warranty (and they do go out sometimes) is by the same people who made your case, so they’re easy to track down. The only big consideration is that you need to get a case that supports the new generation boards (socket 478 and 775 for power users) because the power supply has this special 4-pin plug that any motherboard made in the past 5 years needs or it won’t even boot up. But still, the biggest part of your computer case selection is going to be cosmetics and personal style, so pick one you like!

8. Other Drives. Once the main parts are together and your system’s up and running, you can pick and choose what extras you like or need – everything from CD or DVD burners to MP3 hubs and advanced audio hookups.
With a little smart shopping, and little close attention to quality brands, you can build one affordable, high-performance PC in a snap!

Olive Leaf Extract – Your Body Will Thank You! - Comments Off

Natural ingredients, such as olive leaf extract, are very beneficial to the human body. Every day scientists and researchers are discovering reasons why you should choose to use olive leaf extract daily as a preventative measure, and as an active measure to cure illness. Olive leaf extract can be used to prevent illness and help aid in illness you may already have been diagnosed with.

Why Should I Use the Olive Leaf Extract?

The benefits of the olive leaf and its extract have been studied for many years. In fact, the ancient Egyptians used extract in their mummification and preservation rituals. The olive tree, its branch, leaf, and extract are often referenced throughout the Bible.

Extract Is:

Did you know the first substance to be extracted from the olive is called oleuropein? It was discovered in 1843. Oleuropein is described as a bitter substance found throughout the entire olive tree. This extract is a part of the olive tree that makes it durable. This olive leaf extract is also extremely resistant to bacteria and insects. Olive leaf extract went through extensive laboratory experimentation and was found to be a safe, non-toxic substance. Olive leaf extract was immediately found to be helpful in fighting viruses. Different pharmaceutical companies and scientists used a variety of compounds and measurement compositions of the olive leaf extract in tests. The extract is a “true anti-viral substance because it appears to selectively block an entire virus-specific system in the infected hose. It thus appears to offer healing affects not addressed by pharmaceutical antibiotics” (James R. Priviteria, M.D.).

It’s Beneficial!

As a supplement, olive leaf extract, was first used by health care professionals in the 1990’s. The following is a brief list of illnesses olive leaf extract benefits.

• HIV/ARC/AIDS

• Hepatitis

• Chronic fatigue

• Urinary tract infections

• Shingles

• Blood poisoning

• Diarrhea

• Common cold

• Pneumonia

• And more!

You can find a wide array of books, holistic practitioners, and other online resources for the olive leaf for you to utilize in your lifestyle change.

Dating Advice for Men: Why Some Women Like ‘Em Younger - Comments Off

Demi Moore and Ashton Kutcher. Madonna and Guy Ritchie. Your old high school English teacher and… your best buddy?! Hey, it could happen. These days, if an older woman wants a baby, she can have one.

There will always be the Traditional Tina, scoping out an older fella who’s “on her maturity level” as she likes to think- and who’s willing to play provider. But what of the ever-growing population of footloose women with their crazy cradle-robbing ways? Is this another Maggie May, who, as Rod Stewart tells it, will “wreck your bed, and in the morning kick you in the head?” Or is she some lonely desperado, hoping to latch on to a kinder, gentler generation of men? Here are some insights as to why some women prefer to date younger guys.

Not every girl wants a father figure. Often, a man who’s older than you assumes a protective and paternal role; and to many women, this is a real turn-off. Why? Well, for starters, we already have a daddy! Not to mention, the woman who holds a high-paying exec job Monday through Friday may have some trouble playing Helpless Heather on the weekends. She’s used to taking care of herself, speaking on her own behalf, and yes, even paying her own way without the help of Daddy Warbucks.

Younger men are often better in bed. Ahh, the cruel irony of life. Remember sex with your college girlfriend? That 21-year-old ‘fraidy cat is now a tigress of temptation. Something funny happens to women as they age. Their testosterone levels increase; oddly enough, around the same time that men’s are plummeting. The hormone surge gives ladies a libido lift, which is why women in their 30s and 40s just want to get busy. Meanwhile, our same-age male counterparts are busy studying the inside of their eyelids. Need I say more?

Younger guys look up to an older, successful woman. Let’s talk about those corporate power babes again. Sometimes, it’s lonely at the top… but not when there’s a starry-eyed cutie-pie smiling up at you! There’s something to be said for having a younger guy who admires you for your accomplishments. It makes you feel great, and the sad fact is, you can’t always get that kind of respect from an older man. Baby Boomers may love Donna Reed, but guys born later can understand and better appreciate women who go after their dreams.

The metrosexual male is our cup of tea. Nothing like having your boy over for some afternoon delight and finding out he borrowed your loofah in the shower. The New Generation of open-minded guys is A-OK in our book! This guy can whip up a Thai stir-fry using organic ingredients, grooms his fingernails and his toenails, is politically-correct to a fault, and won’t eat Starkist if it might harm the baby dolphins. To the 40-something who grew up with Archie Bunker, he’s a breath of fresh air.

Girls just want to have fun. Some of us are downright immature, and we’re not afraid to say it! Perhaps “young at heart” is the better term, but the point is this: some old gals really do just want to have fun. Lots of broads who are pushing fifty still rock out at concerts, drive 70 mph with the top down, stay out all night, flip people off when necessary, and generally wreak havoc. That’s not to say we can’t find a man of ANY age to engage in juvenile antics with us… but, well, the younger ones just seem more willing to join in the fun.

Some women are late bloomers. Embarrassing as it may be, plenty of chicks spend the better part of their 20s mired in self-doubt and confusion. Instead of kicking back like they should have, the rainclouds of pessimism hung around to dampen their fun and turn them into neurotic freaks. Ten years later, some of those very same basketcases have mellowed into chill ladies who are just now ready to embrace life to the fullest. If this involves cavorting with young fellas in ways they never thought possible, all the better! Better late than never, right girls?

Guys in their prime are ohhhh, so fine. If you had to choose between a thick, juicy steak and some shriveled up beef jerky, which one would you pick? In the same way that men can’t help looking at supple young girls, we ladies find ourselves drawn to guys under thirty. Younger men have tight bodies, bright eyes, great smiles, and usually a crowd of buddies to keep them laughing and pumped for excitement. What’s the alternative? The balding bachelor with a beer gut who sits home smoking stogies and listening to Springsteen’s Glory Days on repeat mode? No, thanks!

In all seriousness, it shouldn’t matter the age of two people who mutually respect and enjoy each other’s company. We love who we love, not matter what their age or what they look like. If an older woman and a younger man feel comfortable having a relationship, then no one has the right to criticize them. To the lady who simply likes a quick fling with a hot young stallion? Here’s to you, Mrs. Robinson.

Should Artificial Intelligent Robots Be Able to Tell Jokes? - Comments Off

There are obviously degrees of intelligence and if we are to call human beings, intelligent and I hear some people laughing in the background as I say this, but if we use the human brain as our model to design artificial intelligent robots then, well we have our work cut out for us.

Indeed, I would think that a higher-end artificially intelligent robot should be able to think enough to come up with its own jokes that their human companion would indeed laugh at. This subject came up recently in an online think tank when one member, Keith, while considering this subject stated;

“I couldn’t disagree more. Humans expect computers “and robots” to respond with facts not deception, jovial or otherwise. Would you tolerate a clothes washing machine or dishwasher that said the items were clean when they were not? Or wasted your time accepting a command but not executing it, “as a joke”?”

This of course is not what I had in mind really, but this is an interesting point indeed that Keith makes. I was more considering that as humans have companion artificial intelligent robots, android assistants, the human must bond with the artificial intelligent robot and there needs to be an attempt to keep things on a level playing field of respect. Because taking advice from a robot or artificial intelligent machine you must respect it too. And thus you need report.

Now then if your washing machine screwed you over and you could not trust it that is one thing, a negative, yet if you had to live with an artificial person, then that person would need some additional human traits above and beyond mimicking behavior. Humor is something important; comic relief makes sense. Interesting subject no doubt and perhaps you might consider this in 2006.

What Can You Learn From These Business Owners In the Shadow Of Hurricane Katrina? - Comments Off

Here is a Case study of how Online Business owners affected by Hurricane Katrina got through the crisis. We can all learn from their stories.

Business owner 1 ran a very successful online storefront from his home in the Gulf Coast region. He ran his business off a desktop computer. When he heard the Hurricane Warnings he backed his data up using a DVD Recorder. He also decided to buy that laptop he had wanted for so long. He took his DVD back-up and restored it to his laptop and did a few hours of testing to make sure his laptop was operational. After he was sure he had a good backup copy. He Made 3 Copies of his DVD Backup. He mailed one DVD to a relative in Arizona. He placed the 2nd DVD Backup in a Safe Deposit box in his bank and he left the 3rd DVD backup with his laptop computer. On Sunday when the mandatory evacuation order came he moved his home desktop computer to his attic. He grabbed his Laptop and a suitcase and drove to Safety. Today 2 weeks after the Hurricane Katrina Disaster he is running his online business from his hotel room on his laptop.

Our second business owner lives in the St. Louis Area 700 Miles from the Hurricane Katrina Disaster Area. His online business was hosted by a New Orleans based company. Hearing about the Hurricane he decided to log into his CPanel and backup his web site. He downloaded the backup to his personal computer. A few days later when his Host went down because of the Hurricane Katrina he simply found a new host restore his web page from his backup to his new host. Changed his Name Servers with his registrar and he was back in business a few days later when the new Name Servers Propagated across the Internet.

You got me both business owners are fictional, However the stories they tell are important. You need to be prepared for a disaster. In both cases our business owners were able to recover from Hurricane Katrina by listening to the warnings and doing make the required preparations. What if there had been no warnings? What if this had been a Fire, Earthquake, Theft or Terrorist Attack? Neither of our business owners would have been prepared. Are you prepared? Consider this your warning. Backup your data. Place a copy of that backup in a safe place nearby. Mail a copy of that backup to a trusted friend or associate 1,000 miles or more away.

Learn The Easy Way-From Other People’s Mistakes - Comments Off

We’ve all been subjected to awful speakerssome are boring, others are sanctimonious, a few are tedious. The one thing all of these rotten speakers have in common is this: listening to them is pure torture and all you can think about is how you will never get the last thirty minutes of your life back.

Most of us tune out quickly once we are subjected to a lousy speaker. We pretend to take notes on our Palms only to check our email or add items to our To Do List. We daydream about our summer vacations. Occasionally, we literally fall asleep.

The master communicator does none of these things. Instead, the master listens and watches the poor speaker AND watches the other audience members carefully. The master understands that the more you know about how other speakers lose their audiences, the less likely you are to lose your own audience the next time you have to speak.

Only by watching and listening to dreadful speakers can you break down, step-by-step, exactly what their mistakes are. Once mistakes are isolated, they are easier not to repeat.

The essential ingredient that master communicators must have is self-awareness. The masters must be able to see other people’s blunders and acknowledge that they too have made the same mistakes and are likely to do so again and unless they remind themselves constantly not to do so.

For example, most of us have sat through countless business presentations from executives that are incredibly boring because the speaker lists one abstract fact after another, quickly. The speaker races through 157 key developments that happened in his division during the last 6 months. Not a single example, story, anecdote, vignette or case study is offered, just a quick regurgitation of abstract bullet points.

We’ve all been there, and yet most executives planning a presentation make the exact same blunder when creating and delivering their own speeches. They have no self-awareness.

Master communicators are not necessarily any smarter or harder working than average or boring speakers. The only difference is that they ask themselves, “Why would I want to submit someone to the same thing I hate myself?”

When given the option, I always choose to watch another speaker by standing at the front of the room and on the far left or right side of the room. This allows for a clear and unobstructed view of the speaker and the rest of the audience without being distracting to either. From this position it is easy to see when a speaker is buried in notes, gives insufficient eye contact or ignores whole sections of the room.

Take note when you hear other people speak; your notes should include info on subject matter as well as presentation technique. Your notes may include entries such as this:

“Sally held everyone’s attention brilliantly for the first five minutes as she gave us a blow by blow description of her battle with the account executive at the Acme widget company, but the second she launched into her slides, the energy went out of her voice and the room. Five minutes later, I noticed everyone was slouching and shifting in their chairs uncomfortably. After ten minutes of her slide presentation, I could swear I saw Dickinson nodding off and then catching himself and pretending to mask his snore with a throat clearing.”

Everyone makes speaking mistakes. The master communicators reduce theirs by learning from yours. You can too.
